PLAN DETAILS: The membership at Osmosis Day Spa Sanctuary allows you to experience a treatment(s) every month, Sunday through Friday (excludes Saturdays & Holidays,) for 12 months at a discounted price. Simply complete this agreement and pay by standing order. If you don't use your membership credit one month, you keep your unused service(s) for any future month within your 12 month membership. Services not used within membership term may be used within 60 days after membership completion. Unused services will be forfeited after that time.

ADDITIONAL BENEFITS INCLUDE: Complimentary sound therapy session in a hammock with any service*(Member Only -$22 value) 50% discount on pagoda massage upgrade* 10% off all regular priced skin care and Boutique items anytime. 10% off all regular priced services Sunday through Friday (excludes Saturdays & Holidays - Member Only) Unlimited access to the meditation garden during normal business hours, weather and light permitting. 24-hour cancellation notice required versus 48 hours for non-members. A package series will be deducted if notice is not given. *if weather is permitting and space is available.

SUBJECT OF SALE: In accordance with the terms and conditions set forth herein, please enter our order to join The Vital Health Club at Osmosis Day Spa Sanctuary for 12 months.

MEMBERSHIP: Membership includes your choice of one of the services listed below each month. Signature Cedar Enzyme Bath for two, one 70-min Swedish Massage, or one 65-min Vital Energy Facial.

TOTAL PRICE OF ANNUAL MEMBERSHIP: $2,028
PAYMENT TERMS: The amount $169 to be billed in 12 consecutive monthly payments commencing on the day of enrollment until paid in full. If credit card payment is unable to be processed due to card errors or insufficient funds, the balance will be subjected to a $15.00 per month late fee and/or reasonable collection fees.

GUEST HEREBY ACKNOWLEDGES:

a. The guest acknowledges upon signing this contract that she/he has read this contract and has not relied upon any promise, statement or representation other than as contained herein and hereby acknowledges receipt of a copy of this agreement.
b. This contract may be cancelled within 60 days of signing-up if not satisfied with membership. After the 60 day period, 30-day notice of cancellation is required.
c. This contract may be cancelled for compelling medical reasons. A written statement from the guest’s medical provider is required.
d. Osmosis Day Spa Sanctuary reserves the right to cancel the contract if the Guest demonstrates inappropriate behavior towards any employee of Osmosis Day Spa Sanctuary.
e. The Guest acknowledges she/he has provided accurate information on spa consultation form and the services in this contract are recommended for her/him.
f. It is expressly understood by the member that these services are not transferable.
g. Osmosis Day Spa Sanctuary does not guarantee that appointments are always available but will make all reasonable efforts to schedule the Guest’s monthly appointments. It is recommended that appointments are scheduled at least four (4) weeks in advance.
h. Contract may be terminated if the Guest moves more than 60 miles away from Osmosis Day Spa Sanctuary. Written documentation to verify new housing must be provided 30 days prior to cancellation date.
i. In the event of an early termination, the Guest will be billed for the full price of all services received as part of the Vital Health Club at Osmosis Day Spa Sanctuary.

FORCE MAJEURE: Osmosis Day Spa Sanctuary shall not be liable for failure to provide services caused by labor disputes, strikes, war, fire, flood, accident, paper or other material shortages; or any act of God. In such case Osmosis Day Spa Sanctuary may extend the time for completion of Agreement or credit a prorated amount to the Guest in its discretion.
